The Seller Agrees:

This puppy has been well taken care of, looked after properly, and fed top premium food. This puppy has been wormed and given its intitial 5-way shot. It has also been vet checked to insure its health. We offer benefits listed below to our clients who purchase puppies from Down Puppy Lane Kennels.  

 

GENETIC HEALTH GUARANTEE

There will be a 1-year health guarantee on the puppy.  If the puppy should come down 
with any genetic issues; such as diabetes ect., an explanation from 
the vet would be necessary for this guarantee. We will not refund any money paid for the 
puppy considering this genetic problem, but we will replace the puppy with a new 
healthy one.

We, here at Down Puppy Lane Kennels, have strived to give you a good, healthy, puppy.

THE BUYER AGREES

That this puppy will be given adequate and proper shelter and housed securely, shall not be 
neglected, allowed to roam free, or mistreated in ANY manner.

The Buyer agrees to not hold Down Puppy Lane Kennels or land owners (Burton and Rebecca Hartley) accountable for any accident that may accrue on property while visiting the kennel to see or to purchase a puppy.

1st Shot (5 way)

Vaccine canine distemper-Adenovirus Type 2-Parainfluenza-Parvovirus Vaccine

Down Puppy Lane will not guarantee the health of the puppy if given a 7 way shot before 
1 year of age.

 I ______________________ (buyer) understand all the contents in this contract and by

signing the above line release the seller from any expenses for vet bills or any other 
expenses incurred for my puppy through it's lifetime. I will not ask for the purchase price 
back to help pay these bills.
